About The Job

Provide holistic support to adult cancer patients and their caregivers. The role involves providing psycho-social and emotional support, connecting clients with appropriate resources, coordinating care, and contributing to programmes that improve the well-being and quality of life of cancer patients.

Roles & Responsibilities

As a Cancer Navigator, you will:

  • Provide support to clients according to their individual needs and circumstances, including those who are newly diagnosed, undergoing active cancer treatment, receiving palliative or end-of-life care, and cancer survivors.
  • Provide psycho-social and emotional support to cancer patients and their caregivers.
  • Connect clients with relevant resources and services, including financial assistance, food rations, transportation, mobility equipment and medical consumables.
  • Follow up with community partners to support continuity of care following referrals.
  • Refer clients for counselling, spiritual support or mental health services when appropriate.
  • Provide information or coordinate psychoeducation sessions with medical and healthcare practitioners to help clients better understand their cancer diagnosis, treatment and potential side effects.
  • Maintain regular communication and relationships with clients and caregivers to understand and assess how they are coping throughout their cancer journey.
  • Advocate for the needs of cancer patients and contribute to cancer awareness through community programmes and publicity campaigns.
  • Maintain accurate case documentation based on the interventions and support provided to clients.
  • Carry out administrative and coordination duties required for programmes and support groups.
  • Contribute to the planning and implementation of programmes and support groups according to the needs of cancer patients at the centre.
  • Facilitate support group sessions and programmes for cancer patients and their caregivers.

Education and/or Work Experience Requirements

  • Degree in Counselling or Psychology.
  • Demonstrated experience in case management and supporting individuals affected by illnesses or multiple adversities.
  • Ability to demonstrate warmth, empathy and sensitivity when supporting individuals affected by illnesses or multiple adversities.
  • An interest in providing support to individuals and families from lower socio-economic backgrounds.
  • Willingness and ability to work with clients across different age groups, including children, adolescents, young adults, middle-aged adults and older adults.

Other Requirements

  • Able to coordinate logistical requirements for both in-house and external programmes.
  • Able to work on weekends when required by the supervisor, including for client outings, support group sessions and outreach programmes.
  • Able to travel for home visits.

$3,500 to $3,820 Monthly
